We use dokuwiki. Pages are developed on an internal version on our intranet and then copied after review and approval to an external mirror. We put our documentation, faq, troubleshooting and help information on there. It's just had its 10th anniversary, 50,000 topics, 500+ users.

Ideally, anyone could enter a question and a response, including tech support, tech writers, engineers, and customers. We would include a check/approval step before it is published.

We're NOT in the business of developing software, and so don't produce html help or anything like that. It would be freestanding, probably available through the corporate website.
