My two cents is this: I've been editing book length manuscripts for 
years. Since everyone I've ever worked with defines a page differently, 
the onus is on you to provide what you're going to bill off of. In other 
words, I tell the client in an email, and again in a small contract that 
edited pages are double spaced 8.5x11 pages with 1.5 margins.

Having sued a number of people in my career, you don't need to pay an 
attorney to create a demand letter for you. I wrote one in January and 
the deadbeat I won a $7010 judgment from worked out a payment schedule 
with me after receiving the demand letter. However, I do believe even a 
lawyer would say this, that a demand letter won't go out now if you have 
a 90-120 net. A demand letter is usually sent after someone has 
neglected to pay or after a claims court win.

I think being as polite as possible until you receive some payment or 
120 days rolls by is the best bet. Given your problems with this client 
I wouldn't be surprised if they don't answer your questions, and then, 
as someone else suggested, sending an invoice for the full amount is the 
best thing to do.
