I dunno... I'm not a pro book editor, but it seems that since "manuscript page" and "book page" are both variables controlled by the customer, not you, you need an absolute measurement that works for both parties.

If clients catch on that you are billing per manuscript page, you will eventually start getting manuscripts on foolscap in 6-point type :-)

Suggestion: "My rate is $X per page... a page consists of n words, as reported by MS Word..." Then you can negotiate n.
