The simple explanation is that the common cause for this problem is calling
a 32 bit help file from a 16 bit application. The original calls for help in
16 bit applications are to winhelp.exe, the 16 bit help engine, therefore NT
will launch the 16 bit engine. As a result it cannot read the 32 bit file.
On Windows 95/98 winhelp.exe is just a stub that launches winhlp32.exe that
has thunking code which allows it to read both 16 and 32 bit help files.
However, due to NT's security requirements it won't run thunking code so it
launches either the 16 or 32 bit help engine as requested.
