Our sign-off sheet contains the following:
Author
Date sent out and date required by
Doc title, ref number and version
Reviewer(s) - Title, Name, space for signature and date
Comments box - comments are generally marked up on the doc itself but
this field allows for the author to make comments - is xyz on page 30
right - and for the reviewer to make general comments/pointers - e.g,
see pages 4, 9, 35 etc
Author sign-off box stating that comments have been incorporated
