Interleaf can do all of these (somewhat surprisingly, as Interleaf's
Word input filter does not have a reputation for robustness). The index
entries come in clean and usable, or at least they did back in Word 5.0
days. Someone should confirm this with Word 97 before anyone goes out
and spends what it takes to get up and running with Interleaf.
