Kevin, if I could have listed all the cost categories a doctor would
include in the revenue requirement calculation, I would have, and you
are right to have enquired, because I did try.

But I finally admitted to myself that it would only hurt my cause to
continue. You know how I am loathe to forego an opportunity to inspire
others by demonstrating my elite skills as a know-it-all. But it was
late, I was tired, and the list was still inexhaustive.

So rather than paging my diffuse network of 24x7 know-it-alls, and
together assembling the definitive list of all costs of doing business
as a doctor, I decided to fold that tent and steal away into the
shIfting sands of more important things that know-it-alls do with their
time.

Speaking of which, I don't know why anyone is still reading this thread,
as I have already posted everything that needed to be said on the subject.
