You go to a doctor who has no overhead?

Most of us (even here in Canada, with socialized medicine - where the
government takes our health-care dollars, then pays a portion to the
providers) are paying the practice or the clinic.
The practice or clinic then pays the doctor, the receptionist(s) the
nurse(s)/assistant(s), the contracted services that they employ (not
limited to cleaning services, accountants, bookkeepers,
billing/collection services, lawyers, and on and on).

So, for the rest of us, the business is billing us on whatever model it
chooses (or the goobermint chooses for it) to operate. We don't get to
see, directly, how the actual doctor is compensated.

Only some people in this list are working that kind of arrangement
(where they are running their own multi-person business and are either
the owner or a partner). The rest are
