The problem isn't in using any of those formats for file names. It's
in *referring* to those files once one of those formats has been used
(and yes, we get to blame Linux ;-), which is case-sensitive). AFAIK,
it's easier to convert file names, hyperlinks, TOC references, etc. to
lowercase so that everything works.
