Let me expand. By separating structure and length of paragraphs from formatting, I was referring to the fact that, most of the time, you need to exercise considerable judgement to get these things right. Leaving them to XML is theoretically possibly, but usually only if you're satisfied with rather mechanical results. For XML to be worth the time to set up, the material has to be extremely consistent in structure; it might work well in API documentation, for example. Without such consistency, you'd have to spend so much time designing that it would be counter-productive to consider using it in the first place.
