A good place to start is the Online Technical Writing Manual. It has a section on indexing:

http://www.io.com/~hcexres/tcm1603/acchtml/indexing.html

This is pretty good textbook. It's part of the Technical Communications program at Austin Community College.

The main TOC for the book is here:
http://www.io.com/~hcexres/tcm1603/acchtml/acctoc.html

"An enhanced hardcopy version of this online textbook entitled Power Tools for Technical Communication by David A. McMurrey is now available through Thomson Learning/Heinle Publishers. Use ISBN 0-15-506898-0 to get a copy"

No affiliation with the book or the program.
