I was hired as Technical Writer. After a year or so, my boss said, you are doing work that is on par with a Senior Technical Writer. Here is your promotion.

IIRC, it involved acting more autonomously, taking on bigger projects, and kickin' ass.
>>Did you have to pass some exam, or was your performance rated against established parameters?
No. This ain't the series 7.
>>Did you have to be a writer for X number of years?
I think 5 years should be a minimum time frame, but it is not cut and dry. Each company defines their own criteria.
>>Did you receive a salary increase, and if yes, what percentage?
Approx 20% increase in pay.
