The approach to leave the file in a "common" folder and add it to books from
there is a good one... one file to maintain. Yes, you will run into
Header/Footer issues, but that's where File > Import > Formats comes in
handy. When you publish a book, before you generate and update, just import
the correct page layouts (master pages) into the commonly-accessed file.
After you do that, gen and update the book and you should be good to go,
barring any bad x-ref anomalies you may encounter. It's a simple process,
and one that doesn't add much to the publishing workflow, as long as
multiple people don't try publishing books that all reference the same file
at once. ;)
