Apparently, "upgrading" to Office 2000 can disable this button. It was
still on my toolbar, but it didn't work until I re-installed Acrobat.

When you install Acrobat 4 or 5, and you already have Microsoft Office
installed, you get a neat little button in your Office application toolbars.
When you click the button in Word, you get a Create PDF dialog that allows
you to specify all kinds of neat stuff, like which heading levels should be
converted to bookmarks in the PDF file, how you want Table of Contents
entries hyperlinked, and so forth.
