Nobody's said this so far, so I will. This feature is (optionally)
automatic if you use FrameMaker. Choose File> Generate/Book, then the
Standard Index option. In the next dialog, tick the option "Create
Hypertext links". The document's index is then created as a separate file
which you then Import (File> Import> File... at the appropriate point at
the end of the document. The page numbers are hyperlinked to the index
entries' sources in the text both with the document, from within the
separate index file, and from within the index in the PDF when you Save
As... the document as such.

The hyperlinking is not just convenient for the reader, but great for
reviewing and fine-tuning entries before going to print.

The same facility is available for TOC.

Of course, if you're not using FrameMaker, this is of no help, but it'll
serve as a consideration next time you have a chance to decide on the right
tool for the job.
