Do you give a product questionnaire to your SMEs?

I'm thinking about creating one. Even if the SME doesn't fill it out, I can
use the questionnaire to interview the SME face-to-face.

If your answer is yes, how was it received? The people here hate to fill out
forms. Did you find it useful to obtain information?

I'd really appreciate suggestions or comments because I need to improve my
subject gathering method.

I'm pretty bad at just writing a manual based on product and functional
specifications without interviewing anyone. I think that I rely to much on
markups to my rough draft.
