Regarding left and right headers and footers for single-sided PDF printing:

What we did with the same problem, was to basically just use the "first" and
"right" master pages. First is used for the first page of all chapters which
is a right-hand page anyway. Both have the headers and footers flush right
so if our users print the docs from Acrobat and put them in a notebook, the
information is on the outside edge for easy flip-scanning. We also changed
from a 7.5 x 9.5 inch format to an 8.5 x 11 inch format so we would be
wasting our customers' paper.
