The virgule in "and/or" is an "OR," not an "AND," and "OR"
isn't exclusive unless explicitly defined as such.
So "and/or" means "either one or both."
Having said that, though, I can't think of a more awkward and
obscure way to say "either one or both" than to use "and/or."
I can understand it, but I don't have to like it.
