Adobe has a free utility that you can download from its web site. It's called

Adobe PDFMaker for Word 97 and provides enhanced features when creating PDF
files from Word 97 documents. This will convert headings to bookmarks and
make cross-references into links.

As far as I know, they don't have a similar utility for previous versions of
Word.
