Subject: Request for Documentation Form

I would like to create a "Request for Documentation" form for use in my
company. Currently, requests for documentation are made informally - using
e-mail or voice mail. Does anyone have any ideas on what I should
include? Do you have any templates?
Any help would be appreciated.
