Not entirely true, actually. More than one writer has discovered
their 'design' as they've written. It may have been semi-conscious
from the start, but they revealed it to themselves incrementally, not
in one preconceived plan.

And sometimes it's a complete surprise. Tolkien, while writing Lord of the
Rings, remarked in a letter to a friend that he (Tolkien) was very curious
about who this fellow Strider was who just popped in to the story!
