PageMaker 6 (don't know about earlier) and above, like FrameMaker, can
manage separate documents as a single book, and can compile a TOC from
the member files of the book. You can designate the starting page number
of a document, so your frontmatter pieces can have their own numbers, the
first file can start at 1, and the following files can be autonumbered.
