I only use the 'up- and down-style' for the title of the manual.

All other headings are downstyle. They look clean and easy to read. Mixed case
(up- and down-) seems to add clutter to the page.

You can emphasize the headings with bolding and a larger size; even just using a
san serif font helps it stand apart from the text (assuming you use serif for
the text).
