There is a relatively accessible description of entity-relationship
diagrams in _Diagramming_Techniques_for_Analysts_and_Programmers_ by
James Martin and Carma McClure (1985, ISBN 0-13-208794-4 in hardcover).
I find this book a handy reference for the whole range of diagrams it
covers, although Martin's tendency to promote his own ideas gets in
my way at times.
