Most start ups hire with individual contributor titles and then upgrade them when they get cleared to hire more people. But I once worked for a start up where the first person hired for each function got a business card with the title "Manager," the reasoning being that if the company made it, that person would be the one hiring and managing everyone else in that function so why spend extra money to replace all the unused business cards?

There are also people who manage process but don't necessarily have people reporting under them (project managers, for example). "Content Manager" is probably one of those.
