When you add an image to a Word doc, the image's description defaults
to the filename. When you PDF the file, the description becomes the
image's alt attribute.

If you would like to use different text (which is suggested for
accessibility reasons):

1. Right-click the image and select Format Picture.
2. Select Alt Text (bottom option on the left).
3. Change the description to reflect the purpose of the image. (Ignore
the title...for some weird reason, it's not used anywhere.)

I'm using Word 2007, and found the Alt Text option in the Format Size (instead of Format Picture) option.
