It's probably fill the whole page, "block" style... I COULD contact the journal but I need to get this paper off to my client today. I've told them it would help if I had an example of the journal to work with as well. The instructions are definitely poorly written. They state numerical references but don't specify WHICH style - in parentheses or using superscripts (different journals use different styles).
