Oh, man, that's a sore point for me. I learned at one client that very frequently, when one of their systems was shipped, the customer's Receiving Department opened the box, removed the manual and stuck it on a shelf somewhere or even THREW IT OUT, and sent the system on to the technician who would actually use it . . . 

That absolutely drove me NUTS. 

The best procedures could have been planned, and the best documents in the world could have been written, but they may never have made it onboard the rig.
