I prefer to index as I go along. This has several advantages:
- Indexing is a nice change from writing and editing sometimes.
- I can actually use the index to find things that I know are in the doc
somehwere.
- Using the index alerts me to things that I'd like to find but which aren't
in the index.
- If I leave the indexing to the end, it tends to get pushed aside in the
rush.

The only disadvantage is that, as Kevin has said, it takes a bit more effort
to be consistent from one day to the next.
