It is just problem that is at the heart of many project failures.
 
The first three things that should be set up for a project are the  FOLLOWING 
CRITICAL ENTITIES
 
1) Project Manage
2) Quality Assurance
3. Configuration Management.
 
All else gives body to these three.
 
Very Simple.
 
        PLAN your work --  document it
 
        WORK your plan --  document it
 
        TRACK your Plan  --  control it
 
Any other process falls flat as you go through the process.
 
 
People:  (In my opinion the most important people/functions
 
External
       Client/Customer
Internal
       Stake holder
       Project Manager
       QA Manager
       Configuration Manager
       TECHNICAL WRITER/DOCUMENTATION GROUP 
                (should  be obvious, but sometimes the last one in, 
                        and  the one who gets to 'take the heat')

Remember Murphy's Law, whatever will happen, will happen, and at the most  
inoportune time.
 
REMEMBER Y2K.  
      VERY BAD MANAGEMENT DECISION 
                (only  use thel ast two digits of the year, you won't be 
around (told me in  1973).
