My favorite reference for making sure surveys hit the mark is actually an
undergrad textbook: Survey Research Methods, 2nd Edition, by Floyd Fowler,
Jr.

The chapters on designing good questions and evaluating questions are
excellent resources in making sure the data you gather measures what you
intend.

By the way, there is a 3rd edition available.
