Well yes you can, but you would have to program it.

Although docbook does have a callout mechanism that helps you annotate code
listings :-)

My suggestion with images where you need callouts is to use SVG and then
dynamically build the output formats such as PNG at build time. This way you
can also include SVG directly into the docbook mediaobject and save on doing
the source editing and exporting.
