Edgar D' Souza reiterated:

"Anyway, since you apparently possess knowledge that is far beyond what
us ordinary mortals possess, perhaps you could tell us: Is there a way
of marking up text with DocBook, and then rendering with a particular
toolchain+stylesheet to produce an effect similar to that of a callout
in Word or Frame?"

While I am not Eric, may I ask a question?

Since Frame *can* create quite nice DocBook instances, and since it also
handles illustrations with callouts, what is the question?

I'm afraid it escapes me, but then my own knowledge (while not beyond
ordinary mortals) found Eric's comment to be on point.
