The best thing to do is to start with a Doc-To-Help template, copy it,
rename it if you wish, and then modify it to match what you want. An easy
way to do that is to use Word's Organizer to copy styles from your existing
template of choice to the copy of the Doc-To-Help template. I do recommend
working with the default style names, especially the various bulleted and
numbered list styles. Just modify them to match the formatting you want.

Because there are separate templates for each output Help target (including
the printed manual), you will need to add styles to each, if you add them to
the source document template. I recommend keeping the formatting of the
styles in the source document template the same as the printed manual
template, but you do not have to.

I recommend working with copies of the Doc-To-Help template because there
are various toolbars, and routines that are associated with each. The
templates are really very flexible (as are all Word templates), but you do
have to remember that there is a different one for each target.

I like DocToHelp but it is forcing me to use one of their embedded
templates. I tried using one made in house but it did not like it. Maybe
I have not yet spent enough time with the tool to see what format it wants
for templates.

I admit though, that the tool is quick in producing its output formats. So
far, the defeciences I have identified with D2H are:

- Version control for multiple users (check in and out)
- Word or HTML as main editors
- Rigiid PDF output format, though fast

Also, I have not been able to get my head wrapped around conditional text.
Say replacing a Word 'Product X' with 'Product Y' based on a build
condition. I know it does it, but how.
