Ideally I'd like not to have to think about the bookmarks at all, just
generate them automatically when publishing to pdf.
Am I better off looking at an external plugin for this? Any
reccomendations, or things to avoid?
I think you just need to tweak style definitions, conversion settings, etc. At most you might need a macro in Word to do some cleanup.
