Paul,

It sounds as if you are drawing some of the elements outside of the
anchored frame that the image is in. That's why some things stay with
the image and some don't.

When you import an image into Frame, it has an invisible border around
it that appears as a dotted line when you select View > Borders. That
dotted line border is the anchored frame. Stretch the borders of the
anchored frame until there's enough room to draw your callout arrows.
Then the items you draw inside the anchored frame will stay with
whatever else is there. If you draw things outside of the border, the
items will, by default, be put as objects on the page, and they won't
flow with the text.
