It's dead easy to create an interactive form with Adobe Director,
which is part of
the Creative Suite bundle; I haven't checked on its availability as a
stand alone.
It'd be logical for it to be included with the full version of Acrobat, but ...

Anyway, Director reads in a .pdf and automatically generates the fields that a
user would fill in. Part of the form creation is specifying how the
information is
processed - printed, emailed, etc. It requires a bit of tweaking, but
it's in the
order of a few minutes, not hours or days. I've been using it to fill
out all kinds
of forms that I have to download and process. Beats printing them and filling
them out by hand.

Even without Director in the loop, emailed info should be really easy to parse
into a database or Excel -- I'd think that each field would be a
discrete piece of
information that would just pop into place.
