You can set up an Acrobat form on a web site such that when someone
fills in the data and clicks Submit, the data from the form is put
into a database. I created a proof of concept for this very
functionality some years back. You can see it in action here:

Interestingly, when I tried accessing this just now from Firefox, I
got a message indicating that I couldn't do that operation. I'm not
sure what's up with that, but it works from IE. I am pretty sure I've
accessed it from Firefox in the past. But anyway....
