Yes.

I was doing a project for a Major International Corporation,
and we were using something akin to DFDs. "So," I asked,
"the input here comes from the XXX job that's run overseas
at YYY. How can I find out about that?"

Oh, call so-and-so. Well, so-and-so isn't on that any more
but recommends what-'n-who. What'n-who says, "They're planning
THAT?" THAT'll never happen. Shortly after my discovery
of this bad situation, pretty much the whole division got canned.
