"It happens" only when there's no documentation process. Using Word, for
example, it's easy to set up a process to ensure that the Index has been
properly updated -- along with the TOC, headers, footers, etc. -- before the
document is rendered read-only. I assume that the process is equally
straightforward in FM and other apps.
