A new Microsoft Knowledge Base Article (833973) discusses the following
problem: "When you have a document that uses a Table of Contents that is
based upon styles, and you apply direct (manual) formatting to text that is
formatted with a Heading style, the Table of Contents may take on some of
the direct formatting when the Table of Contents is updated."

Microsoft offers three workarounds, but fails to note the obvious solution:
Don't use manual formatting! Geez...
