Troubleshooting Word RD Field for TOCs

Driving myself nuts here. I'm generating a TOC using Word's field codes. 
The document looks like this:

{ TOC \o "1-3" }
{ RD chapter1.doc }
{ RD chapter2.doc }
{ RD chapter3.doc }

All documents are stored locally (C drive) in the same folder). When I 
toggle the field code to see the actual TOC that is generated, all of the 
chapter headings appear (Headings 1-3), but the page numbers each heading 
in chapters 2 and 3 say "error. bookmark undefined." I just don't get it. I 
have tried updating field codes, but that doesn't seem to work. Also, I'm 
only able to generate the TOC when all of the documents are open.

Anyone have any ideas for getting those darn page numbers to appear for 
chapters 2 and 3??? I haven't used this feature in a long time, so maybe 
I'm missing something obvious. (I have checked Word's online help.)
