I don't know whether this is the right information you want because your
mail says that the purpose of the document is to troubleshoot servers but
you have named the document as a server test plan. If you are looking for
a template for documenting possible errors/problems and troubleshooting
instructions, use a format like this:

o Server specifications
-Storage
-Memory
-Load
-Utilization
-Backup policy
-Devices connected to the server

o Network specifications
-Details of workstations and peripherals
-Approximate utilization of resources
-Resource allocation details
-Shutdown and restart procedure in case of emergency

o Details of support contacts
o Problem ID/description
-Explanation
-Symptoms
-Troubleshooting procedure

o Common error messages and description

If you are looking for a test plan or test case template, you can
download a template from any of the below links and customize it to suit
your requirements:

http://www.silkroad-net.com/pdf/test%20plan%20template.pdf
http://www.qacity.com/Test+Types/Load+Performance/Links/Load-Performance+Test+Plan+Template.htm

Goooooogle for more.

Hope this helps!
