Thanks for this very interesting discussion. There are a couple of books
that that I've been exploring that may be of interest. Both are very
pragmatic but substantial and focus on project management -- not sure if
that is exactly what you're looking for, but they are both worth a read.
Process Quality Assurance for UML-Based Products by Bhuvan Unhelkar
Addison-Wesley Object Technology Series
This outlines quality and project management processes for software
development using the UML. It includes a CD-ROM with various templates for
documenting development.
Service- and Component-Based Development using the Select Perspective and UML
Apperly, Hofman, Latchem, etc.

This is a presentation of best practices for software development based on
proprietary software from Select Business Solutions. Even if you don't have
their product the book offers a very good discussion of using the UML to
drive project management. They do offer some interesting products, in
particular Select Process Director, which is a project management support tool.
