This reminds me of one of the best examples of on-the-fly (literally!)
documentation changes in history. On Apollo 13, one of the astronauts
placed a big "NO!" sticker on the switch that would have disconnected the
LEM from the command module. He had nightmares about being so cold and
out-of-it that he would accidentally release the LEM with the other two
astronauts still in it. So he put this big sticker on that control.
Looks like it worked.
