John, for basic back-of-the-book indexing software, you may want to have a look at
wINDEX <http://www.abbington.com/holbert/windex.html>. Other dedicated indexing packages are

Macrex <http://www.macrex.com/>
CINDEX <http://www.indexres.com/>
SKY Index <http://www.sky-software.com/>

These packages are largely used in the book publishing industry to create back-of-the-book indexes. These indexes are usually created as text or RTF files and often have to meet specific formatting requirements.

If you are creating an embedded index, you can write and edit your index as a back-of-the-book index, sort it in page number order, and then embed the finished index into your document files. This approach gets around the problem of editing embedded index markers/fields while being able to use the timesaving features of dedicated indexing software. For example, you can double-post an index entry heading just by pressing a function key.
