For anyone who has been around a while, Hewlett Packard made such an application in the mid- to late-90's, but I can't find reference to it anywhere. This HP application built tables of contents, added headers/footers, and customized the link-depth that the tool would import into the document. Very cool, but apparently, very defunct.

If you're working with PDFs, there's a product called Quite Imposing that costs a couple of hundred dollars (IIRC), but works very well.

With Word, there's a product called ClickBook that acts as an alternate printer driver and does an amazing variety of things <http://www.bluesquirrel.com/>. The only drawback is that it's not a PostScript driver, so EPS graphics are problematical.

I believe Word 2002 may have some imposition capabilities, but I haven't used it (still on Word 97).
