Just one additional article you might be interested to take a look at.

How can I automatically generate an index in Word?

The short answer given in the article is "You can't, not if you want it done
properly!", and goes into some detail to explain about making good indexes.
Although it is targetted at Word, most of the principles described are
applicable to any platform.
