We used to use nested headings. Now we increase the margin to 1.25 and
start all Heading 1's at the margin and at the top of a new page. All other
headings and text are indented one more inch (2.25"), use the same font but
in decreasing sizes with Heading 3 using sentence case as opposed to title
case and Heading 4 italicized (rare). This allows white space (all Heading
1 begin a new page) but isn't too busy and we can still identify which
heading is which easily.
