Get thee FrameMaker or FrameMaker+SGML. Do not be daunted by claims of steep learning curve... The FrameMaker curve is only as steep as any curve for learning new software. FrameMaker is made to handle long documents... I routinely work on projects that involve importing 30+ Meg of content (generated via a database) into a FrameMaker book, generating lists (TOC, index, etc.), and turning it into PDF - hypertext links and all. It works like a charm.
